Our Childrens Homes provide safe and nurturing accommodation for children and young people who have suffered adverse life experiences. We provide personalised care that meets needs taking account of the child’s background to help them understand and manage the impact of any experiences such as abuse or neglect. We help support each child to develop resilience and skills that prepare the child to return home or to live independently as an adult
We provide a non-institutionalised home with an ethos of home for children. Staff support, encourage and enable growth of self-esteem, confidence and self-worth through positive role modelling, support, guidance and opportunities. Our aim is to protect and promote each child’s welfare whilst treating them with dignity and respect.
We offer a service to young people aged 16-18 who are preparing for leaving care and need to learn independence and self-sufficiency. The focus of our work is to guide and encourage young people to adopt a positive lifestyle that will keep them safe and productive. We provide guidance around practical skills within the household, financial management, work to reduce risk of exploitation and criminality, promote good citizenship, community integration, employment and healthier lifestyles. The work undertaken to support life skills and practical tasks is underpinned by working on building resilience and self-esteem so that young people are not only equipped with the necessary practical skills for life and becoming tenancy ready but are also emotionally prepared for adulthood.
Continuum CQC Homes offer care and accommodation to young people/young adults aged 16 – 30 years. Our homes are registered to support 1-2 clients at any one time.
Our client group is predominantly made up of those diagnosed with mental health needs, learning difficulties and emotional difficulties such as post-traumatic stress disorder (PTSD), self-injury and emotional dysregulation. Our Homes offer long terms solutions for clients who are unable to live independently and who require practical and emotional support to reduce the risk of exploitation and harm from self and/or others. Each home offers a therapeutic environment that is person centred and bespoke in design, putting the client first and foremost in the planning around service delivery. We promote inclusivity, independence, positive choices and improved quality of life.
Continuum has experience of delivering bespoke packages of support that are individualised to meet the needs of the client. We specialise in the transitioning of young people who are exiting Tier4 mental health services into a community-based programme of support – our experience includes working in support of a Community Treatment Order (CTO) or Deprivation of Liberty Safeguards (DoLS)
We offer a wrap-round holistic service adopting a multi-disciplinary framework of working in partnership with Social Care, Health, Education and other stakeholders. We work with young people in a therapeutic risk-taking environment to promote and aid their mental health recovery and independence.
We can also offer assessment support programmes e.g. supporting Mother & Baby assessment, Parenting Assessments and contribution to Court Care Proceedings
